About This Position
Community First Health partners with OneSource to fill a contract Case Manager opening in their adult outpatient mental health program.
Responsibilities
- Carry a caseload of 25–30 adults with serious mental illness
- Conduct assessments, develop care plans, and coordinate community services
- Facilitate linkage to housing, benefits, and crisis services
Requirements
- Active CA LCSW license in good standing
- 2+ years community mental health experience
- Experience with Medi-Cal documentation
